Survey Form trouble. Need help pls

Hello, I am receiving errors for my form telling me " inside the form element, I am required to enter my name in a field with id=“name”. " same with the email. I have the id’s inputted within the form element. If anyone has feedback or tips it would be greatly appreciated.

I’d double check your spelling of “required”.
