Textarea in Survey Form help!

I’m not sure what I’ve done wrong here! I’ve added a textarea into my survey form and used a closing tag, but the rest of my code (checkboxes) appears within the text area that I’ve created. I’ve added the link to my codepen below.

Any idea how to fix this? Any help very much appreciated!
