Create a Form Element again

I keep getting the error message that says - The existing input element should be nested within a form element. My line of code in question is below, someone please tell me why this is not considered nested.

I’ve edited your post for readability. When you enter a code block into a forum post, please precede it with a separate line of three backticks and follow it with a separate line of three backticks to make it easier to read.

Ok clearly I am very green lol, - I will make these changes if I have to post again. But what is wrong with the code? I cant figure out how to get to the next step or what I should change.

The attribute values for type and placeholder should be inside quotes, and they have to be normal double "" or single '' quotes.

After adding questions to both type and placeholder attributes I am still getting the same error message. That reads, the existing input element should be nested within a form element.
