Nothing to say in terms of the tribute page, except that I would put more spacing between the cards. Also, it’s a mess on mobile. Try using media queries.
In terms of the survey form, looks good. I would put more space between the checkboxes and the options.

Shall address the shortcomings.
I tested the Tribute Page in Chrome, Safari and Firefox. Resizing viewports adjust content after a split second to intended layout.
Please indicate which browsers did you use, and what viewport sizes rendered horribly?

Sorry, I should not have said it was a mess. I meant that there is overflow from the video, and the cards are very long and overlap. I was using Firefox at 460px, Editor mode.

That’s a great idea. I’ve had trouble making flip cards, but if you can do them, that would look great.
