Survey Form Background Image not working

Ive tried different ways that ive seen around the web to fix my background image problem. i just cant get the image to display at all. i want it to stretch the length of the page.I used dropbox for my image and will post the code used below.

hmm that did work. do you know why i was unable to use the dropbox link? i was under the impression that we were unable to use hotlinks.
