Your form looks good @westluv077. Some things to revisit;

  • Run your HTML code through the W3C validator.
    • There are HTML coding errors you should be aware of and address.
    • Since copy/paste from codepen you can ignore the first warning and first two errors.
  • User’s should be able to click on the label to toggle a selection, not just the radio button / checkbox.
  • Change the cursor to a pointer when hovering over the submit button
  • Do not use the <br> element to force line breaks or spacing. That’s what CSS is for.

A suggestion. Investigate the fieldset element and see what it can do for grouping your radio buttons and checkboxes.

Hey @westluv077. your page looks good.

However there’re some issues you have to address. First of all, I think your html code has too many div elements and it makes it hard to read.
So consider revising it. And delete those br elements.

Provide id attribute for your checkboxes and use label element with for attribute referring to those id's.

Make the project from scratch, with your own code, style and content. Don’t take code from the sample project.
