Welcome to the forums @renato_rox. Some things to revisit;

  • Run your HTML code through the W3C validator.
    • Since copy/paste from codepen you can ignore the first warning and first two errors.
    • There are coding errors you need to address.
  • Don’t use &nbsp; or <br> to force spacing or line breaks. Use CSS
  • Users should be able to click on the label, not just the radio button or checkbox.
  • change the cursor to a pointer when hovering over the submit button
  • Do not use fixed dimensions on elements, use max-width/height and relative units like % and vw to keep everything responsive.
