Hi @Aveolar, welcome back. Your form looks okay. Some things to revisit;

  • Don’t use the <br> element to force spacing. Use margin and/or padding in CSS instead.
  • Make it so users can click on the labels for the radio buttons and checkboxes. Not just the little area.

Responsiveness seems real nice to me. I think it looks pretty good.

I would suggest adding just a little space between the section titles and the labels under them.

Also it seems a fixed position is applied to the logo and ti doesn’t seem quite right.

The text in the instructions block also seems heavy. I feel like it would read easier if it was somehow broken up or line spacing added.
