Your form looks good @bdward42. Some things to revisit;

  • Run your HTML code through the W3C validator.
    • There are HTML syntax/coding errors you should be aware of and address.
      Since copy/paste from codepen you can ignore the first warning and first two errors.
  • Change the cursor to a pointer when hovering over the submit button
  • Make your page responsive. Remember, the R in RWD stands for Responsive
    There’s a horizontal scrollbar on smaller screens

Suggestions you don’t have to incorporate if you don’t want to

  • In the fieldset element change the p element to a legend
  • Group the checkboxes in their own fieldset element also

@Roma thanks so much for the feedback and reminders. I’ve implemented the necessary changes, and I plan on playing around with the fieldset element and checkboxes
