@mjjain It looks nice. I would say that you need to increase the width of the input fields and button to at lease 40px. Another thing is, in mobile view I suggest you decrease the margin and increase the width to at least 96% of survey-form id. You will have to do that in a media query.
Nice job on your form @mjjain. My only comment is to change he cursor to a pointer when hovering over the submit button.
