Survey Form Help Needed

Hi guys, i just finished my survey form and i would like to hear your feedback.

Also, i need help with the following

  1. “Things that should be improved in the future (Check all that apply):” is not properly aligned with the options
  2. “Any Comments or Suggestions?” is also not properly aligned with the textbox.

Any help/suggestions on how i can fix it

Bro I think that you have given the padding value too much and that’s why it’s going way towards right.
Reduce it. Increase the width of the input field using ‘width’ field . Instead of ‘inline block’ you can use grid as it’s easy to work with it. Otherwise it’s really good one.

Try setting the position of each class (checkbox, radio, input area, etc) to relative and adjust the margin.
